Intake Form 13614-C can be completed and printed in advance of your appointment. Please bring your completed copy to your appointment. (Paper copies of the Intake Form will also be available when you check in for your appointment. Please allow an extra 15 to 20 minutes before your appointment to complete.)
? Social Security cards or Individual Taxpayer Identification Number (ITIN)
? State I.D., driver’s license or passport for taxpayer and spouse
? Form 1095-A if insurance is provided by the Market Place
? Copy of 2021 tax returns (federal and state)
? Bank account number and routing number for direct deposit
Income Documents
? W-2 Form, if employee
? Form SSA-1099, Social Security benefits statement
? Form 1099-K or 1099-NEC, if self-employed (ie. Uber driver)
? Form 1099-INT or end-of-year bank statement, interest from checking and savings accounts
? Form 1099-DIV, dividend transactions
? Form 1099-G, unemployment compensation
? Form 1099-R, retirement plan/IRA distributions
? Form W2-G, gambling winnings
? Form 1099-B, stock transactions
Adjustments/Deductions
? Form 1098-E, student loan interest payments
? Educator expenses if you are a licensed K-12 classroom teacher (receipts)
? Detailed listing of charitable contributions
? Form 1098, mortgage interest statement
? Real estate taxes paid in 2022
? Business expenses (if you are a sole proprietor or self-employed) detailed listing of business expenses

? Child care expenses (provider’s name, address and SSN or Employer Tax ID)

This branch shares the Eola Community Center buiding with the Fox Valley Park District. It offers a quiet reading room with a fireplace, study rooms and a family computer lab. The Eola Road Branch has public meeting rooms that may be used for community programs.
